This innovative new Service will provide referred patients with a bespoke, nine month holistic approach to improve their overall health outcomes. Patient empowerment forms the basis of this Service which will be delivered by trained Wellness Coaches and supported by Lifestyle Medicine doctors.

What does the Scheme consist of?

There are a wide range of activities both gym based and class based to choose from for patients that have been through rehabilitation programmes. You will be able to access a wide range of opportunities and these will be available between 4 and 48 weeks of the programme (depending on medical condition). Activities* across Wales consist of:

  • COPD Classes
  • Strength and Balance
  • Gentle Exercise Classes
  • Zumba
  • Aerobics
  • Pilates

If you are thinking of making healthy food choices for yourself or your family, but would like to know more, this course is ideal for you.

Community Food and Nutrition Skills programmes will help you to develop your knowledge and skills of food by learning more about eating well, the benefits of good nutrition to health, creating balanced meals and planning healthy menus.

If you would like to learn more about how to lose weight the healthy way, become more active and to find support and ideas to help you change your eating habits this course is ideal for you.

We offer an 8 week Foodwise for Life healthy lifestyle programme, or if unable to commit to this a one off introductory session to improving your eating patterns.
