Chronic Disease Management
Many people suffer unnecessarily as a result of chronic diseases such as asthma or diabetes. Please see the Doctor or Practice Nurse regularly if you suffer from any of the following chronic diseases.
Attendance at any of our Chronic Disease Clinics are by invitation only.
Asthma & COPD
If you suffer from asthma or COPD (chronic obstructive pulmonary disease), we will regularly monitor your condition and help you to improve your condition yourself. We will offer you a review at least every year with the Practice Nurse but will see you more often if necessary.
Diabetes
We try to help patients achieve good control of their diabetes and prevent further complications by providing regular check-ups and education. If you have diabetes, you will be asked to attend at least annually for a review with the Doctor and Practice Nurse.
High Blood Pressure
If you are taking medication to control high blood pressure you will be asked to come along to the surgery for frequent checkups.
Chronic Heart Disease
If you have angina, or have suffered a heart attack, we aim to provide you with a checkup at least every year where we will review your condition, give you the best treatment and help you lower your ‘risk factors’ to prevent further illness.
Patients who have suffered a Stroke
If you or a relative have suffered a stroke (or mini stroke), we will offer you a review at least once a year, to help assess your condition and treatment.
